1. If you are coming in for a drug screen, physical ($50), tetanus injection only ($55), PPD (TB test-$22), or Rapid HIV Test ($50), please click on “Special Visit”.
2. If you’ve never seen before as a patient (illness/injury), please click on “New Patient”.
3. If you’ve been seen before, please click on “Established Patient”.
To expedite the check-in process, please complete the appropriate fill-in form listed above, print it out, and bring it in.
