Not Covered Under Insurance

  • Physical (DOT/Work/School/Sports):  $100 (Please note that we do not perform annual physicals or pre-op physicals.)
  • PPD (TB Test):  $25 (Patient will need to return in 48-72 hours to get the PPD read.)
  • Tetanus:  $55
  • Tetanus/Pertussis:  $85
  • Drug Screen (Hair/Urine):  $70 (Tests are processed at Quest Diagnostics [SAMHSA-certified lab]. Results come back in 2-4 days.)
